ShopADoc, Inc., (“ShopADoc”), is a secure electronic reverse auction website (“Site”) that has been designed to allow Clients in search of reasonably priced dental services the opportunity to receive such services in a more transparent marketplace. For Participating Dentists, ShopADoc provides an opportunity to provide competitively priced dental services to individual Clients seeking such services. Use of this Site by either Clients or Participating Dentists is totally voluntary. The terms “You” or “Your” refer to either Client and/or Participating Dentist as applicable. The terms “We”, “Ours” or “Us” refer to ShopADoc. In order to use this Site You must agree to this Privacy Policy.
Your Consent
By using this Site, You agree with the terms of this Privacy Policy. Whenever You submit information via this Site, You consent to the collection, use, and disclosure of that information in accordance with this Privacy Policy.
About Children
Clients must be at least 18 years of age to enter, navigate, register, make credit card/debit card payments and/or participate in any auction services. We will not knowingly collect information from Site visitors that are children. We encourage parents to talk to their children about their use of the Internet and the information they disclose to Web sites.
Active Information Collection
Like many Web sites, this Site actively collects information from its visitors both by asking You specific questions and by permitting You to communicate directly with Us via e-mail, feedback forms, and/or the Site’s reverse auction services. HIPAA
ShopADoc is not a “covered entity” under the HIPAA privacy and security rules. Although Our platform collects and provides appropriate access to health care information that You provide, We do not offer any services that would make Us a covered entity under HIPAA. Additionally, although We facilitate the exchange of personal information, which may include health care information, between You and Participating Dentists, We are not performing any direct service for any health care provider and as such would not be considered a “business associate” under HIPAA. Regardless, ShopADoc uses appropriate privacy and security safeguards to protect Your information from improper use or disclosure.
Use and Disclosure of Health Care Specific Information
In order to facilitate a Client’s ability to use the services provided by this Site, it is necessary that Client submit certain personal information that is related to health care services previously provided to Client and/or is related to the health care services that the Client is seeking from a Participating Dentist. One category of personal information that Client will be required to provide for the provision of services by a Participating Dentist is radiographic images and photos. It is Client’s sole responsibility to properly mask Your identifying information during the upload of any radiographic image(s) and photos for later reference. Your failure to properly mask Your identifying information is a violation of Site rules and may affect Your present and future use of the Site. It is Your responsibility to utilize Your smartphone/tablet edit functionality. You assume all liability associated with improperly masking Your identifying information. ShopADoc will retain Your uploaded radiograph(s) for up to two auction cycles. At the close of the first or optional second auction cycle, Your radiograph(s) and photos will be permanently deleted. The Participating Dentist that successfully wins the auction to provide services may capture the radiograph images for later use in providing services to the Client. However, Client agrees and acknowledges that a winning Participating Dentist may request that the Client provide the uploaded image at the time of the appointment. ShopADoc uses appropriate privacy and security safeguards to protect Your information from improper use or disclosure.
Use and Disclosure of Non-Health Care Specific Information
Except as otherwise stated, We may use Your information to improve the content of Our Site, to customize the Site to Your preferences, to communicate information to You (if you have requested it), for Our marketing and research purposes, and for the purposes specified in this Privacy Policy. If You provide personally identifiable information to this Site, We may combine such information with other actively collected information unless We specify otherwise at the point of collection. We will take reasonable measures to prevent personally identifiable information from being combined with passively collected information, unless You consent otherwise. We may disclose Your personally identifiable information to third parties, located in the United States and/or any other country, but only
